Sweeteners and Sugar Alcohols: Limits, Labels and Risks

Sugar alcohols vs. artificial sweeteners: Compare metabolic effects, digestive impact, and health outcomes. Learn which suits your needs.

Two different things get called “sugar substitutes” and they behave differently. High-intensity sweeteners are hundreds to thousands of times sweeter than sugar and pass through with essentially no calories. Sugar alcohols are mildly sweet, carry some calories, and are the reason sugar-free sweets have a laxative warning. Here is what FDA has approved, the intake limits it publishes, and what the cancer question actually looks like.

The short version

What FDA has actually approved

Six as food additives: saccharin, aspartame, acesulfame potassium (Ace-K), sucralose, neotame, advantame. Two more are permitted through GRAS notification rather than additive approval — certain steviol glycosides and extracts from Siraitia grosvenorii Swingle (Luo Han Guo) fruit, also known as monk fruit.

FDA’s overall position: based on the available scientific evidence, sweeteners authorized by the FDA are safe for the general population under certain conditions of use. Note the qualifier at the end — safety is tied to intake levels, which is what the next section is.

They are also potent: 200 to 20,000 times sweeter than sugar, adding few or no calories, and they generally do not raise blood sugar levels.

FDA expresses the practical threshold as number of sweetener packets a person would need to consume in a day to reach the acceptable daily intake for a 60 kg person — worth reading on their page rather than converting yourself, since the packet strength differs by product.

An ADI is a level considered safe for daily consumption over a lifetime, not a danger threshold you cross once. But the wide spread between them — aspartame at 50, neotame at 0.3 — is a reminder that “sweetener” is not one substance.

The one hard restriction

Aspartame and phenylketonuria. FDA states that people with difficulty metabolizing phenylalanine because of a rare genetic disorder called phenylketonuria (PKU) should avoid or restrict aspartame, and requires the product label, which must include a statement to inform people with PKU that the product contains phenylalanine.

That warning line on a diet drink is not general-purpose caution. It is aimed at a specific group with a specific metabolic condition.

Sugar alcohols are a different product

Sorbitol, xylitol, lactitol, mannitol, erythritol and maltitol are used primarily in sugar-free candies, cookies, and chewing gums. FDA describes them as slightly lower in calories than sugar and do not promote tooth decay or cause a sudden increase in blood glucose.

Read “slightly lower” carefully — these are not calorie-free, unlike the high-intensity group. And they come with a documented cost: sugar alcohols can also produce abdominal gas, bloating, and diarrhea in some individuals, and products containing sorbitol or mannitol must warn that excess consumption may have a laxative effect.

One labelling quirk worth knowing: sugar alcohol content only has to appear on the Nutrition Facts panel when the package makes a sugar or sugar-alcohol claim. Otherwise listing it is voluntary — so a product can contain them without a line on the panel.

The cancer question, stated properly

The National Cancer Institute’s wording is worth quoting in full because both halves matter: most studies of the association between artificial sweetener intake and cancer in humans have also shown no increase in risk, although one large cohort study found a slight increase in cancer risk among users of several sweeteners.

That is neither “proven safe” nor “linked to cancer”. It is: the bulk of human studies found nothing, one large study found a slight increase, and a cohort study shows association rather than cause. NCI maintains a dedicated artificial sweeteners and cancer fact sheet if you want the detail.

Do they help with weight?

We searched federal sources specifically for this and found no claim that sweeteners cause or assist weight loss. The closest FDA comes is describing consumer motivation — people choose them to cut down on the number of calories they consume or any of a variety of other reasons. That is a statement about why people buy them, not about what happens when they do.

What is documented is the constraint they are usually deployed against: added sugars capped at 10% of calories, 50 grams per day based on a 2,000 calorie daily diet on the label. Swapping a sweetened drink lowers that number. Whether it lowers your weight is a separate question nobody federal has answered for us.

What we are not claiming

  • That artificial sweeteners cause cancer. Most human studies found no increase; one large cohort found a slight one. We report both.
  • That sweeteners help you lose weight. No federal source states it.
  • That sweeteners are harmful to gut bacteria or metabolism. Widely claimed; not something we could source from an approved federal page.
  • A “cans of diet soda” equivalent for the ADI. FDA expresses it in sweetener packets, so we do not convert it into cans.
  • That one sweetener is safer than another. Each has its own ADI; FDA’s safety statement covers the approved set.

If you are using sweeteners to cut added sugar, that goal is well supported by the guidance and the substitution does what it says on the tin. Just do not expect the sweetener itself to do anything further, and if sugar-free sweets make you uncomfortable, that is the sugar alcohols and it is documented rather than imagined.
